Polish Roots Check is run by Igor Kołodziej, ul. Korotyńskiego 42/39, 02-123 Warsaw, Poland, e-mail polishrootscheck@gmail.com (the "Provider"). The Provider is not a law firm and does not provide court representation.
Not included: court proceedings (a complaint to an administrative court requires a Polish attorney you engage directly), transcription of civil records, PESEL and passport applications, and courier costs of sending your originals to Poland.
During our launch period we accept orders only from clients who live outside the European Union and the European Economic Area.
Prices are in US dollars and include the items listed above. Payment is by card through Stripe. Package: 60% when you order, 40% when the application is ready to file; we file only after the second payment. The review fee is credited against the package if you order the package within 90 days.
You provide truthful answers, the documents you hold, a signed power of attorney and, where the office requires originals, the originals or certified copies. Missing or inconsistent records can delay or prevent a positive decision.
The decision is made by the Polish authorities on the basis of the documents. We cannot guarantee a positive decision or a decision time; Warsaw decisions currently take about 18–24 months.
You may withdraw from the contract within 14 days of concluding it without giving a reason, by e-mail to polishrootscheck@gmail.com or using the model form below. We refund all payments within 14 days of receiving your withdrawal, using the same payment method. If you asked us to start work during the 14 days, we may keep an amount proportionate to the work done before you withdrew. If you expressly asked us to start and acknowledged that you lose the right of withdrawal once the service is fully performed, the right expires on full performance (e.g., a delivered expert review).

If, before filing, we conclude that your line does not qualify, we stop and refund the amounts paid minus the cost of work already done (review $79; translations and archive fees already paid to third parties). After filing, state fees are not refundable.
Send complaints to polishrootscheck@gmail.com. We reply within 14 days. EU consumers may also use the EU's out-of-court dispute resources.
We are liable for the careful performance of the services. We are not liable for decisions or delays of public authorities or for consequences of inaccurate information you provide. Nothing here limits rights you have under mandatory consumer law of your country of residence.
These terms are governed by Polish law, without depriving consumers of the protection of mandatory provisions of the law of their country of residence.
